Service Hours:
Medical and Dental Hours: M - F 7 am - 6 pm; closed 12 noon - 1 pm. Provider on call 24/7.
Intake Procedure:
By phone; appointment necessary.
Fees:
Accepts most Medicaid programs, Medicare, PCN (except North Temple), CHIP, and most private insurance. Uses a sliding fee scale for uninsured patients based on household size and income.
