Services include: primary medical care (men/women's care, pediatrics, prenatal and delivery, obstetrics/gynecology, chronic disease care); and general dentistry (preventative, hygiene, restorative).

General Dentistry
Description:

Provides quality, patient-centered primary care services to vulnerable and underserved individuals regardless of their ability to pay. Services cover dental (general dentistry, preventative, hygiene, restorative care). New patient openings are sporadic; call weekly as openings go quickly.

Eligibility:
Vulnerable and underserved individuals regardless of their ability to pay
Service Hours:
Medical and Dental Hours: M - F 7 am - 6 pm; closed 12 noon - 1 pm. Provider on call 24/7.
Intake Procedure:
By phone; appointment necessary.
Fees:
Accepts most Medicaid programs, Medicare, PCN (except North Temple), CHIP, and most private insurance. Uses a sliding fee scale for uninsured patients based on household size and income.
